AGP, Acquisitions, Corp

The Joint Commission Accreditation was achieved in October 2000. AGP, Acquisitions, Corp., operates three outpatient methadone clinics in Pompano, Hollywood, and Palm Beach Florida. The project was started in January 2000 with a target survey date of November 2000. The scope of the project included three sites, three program components, and forty employees. Ms. Potere was the project manager of the The Joint Commission preparation process with a staff of two. This contract has remained ongoing since the accreditation survey. Billable services have included ongoing participation the Agency's Performance Improvement Process, development and revision of policies and procedures as requested, and development of audit tools based on specific program needs regulatory acquirements.


Broward County Florida
Human Services Department, Alcohol and Drug Abuse Services

The Joint Commission Accreditation was achieved in September 1999, making this one of only twelve governmental agencies in the United States to be accredited by The Joint Commission. Alcohol and Drug Abuse Services is a division of the Human Services Department of Broward County, Florida. The project was started in October 1999 with a target survey date of August 1999. The project was completed on time and within budget. The scope of the project was five sites, four program components, and 250 staff. The program components included a 30-bed medical detox, 100-bed residential program, day/night treatment program, and outpatient treatment programs. Broward Addiction Recovery Center (BARC) is the largest county alcohol and drug abuse in the state of Florida. Ms. Potere was the project manager of the entire The Joint Commission preparation process with a staff of three. This contract has remained ongoing since the accreditation survey. Billable services have included ongoing participation in the Agency's Performance Improvement Process, revamping of the Medical Records Systems at the Booher building, development and revision of policies and procedures as requested, development of audit tools based on specific program needs and regulatory requirements, interfaced with the Agency's management information system. Continued participation and attendance at Performance Improvement other committee meetings. Continued monitoring to ensure compliance with The Joint Commission, DCF, AHCA,and Medicaid standards and billing recoupment. This project is ongoing through 2006.


Broward County, Florida
Human Services Department
Family Success Administration Division

The Joint Commission Accreditation was achieved in October 2000, making them one of only twelve governmental agencies in the United States accredited by The Joint Commission! Family Success Administration is a division of the Human Services Department of Broward County, Florida. The project was started in October 1999 with a target survey date of November 2000. The scope of the project was four sites, five program components, and 100 staff. The program components included case management and psychotherapy for homeless services, refugee services, psychotherapy services, and emergency assistance services, plus three regional family success centers. Ms. Potere was the project manager of the entire The Joint Commission preparation process with a staff of three. This project is ongoing through 2006.


Columbia/Deering Hospital
Behavioral Health Services

The Joint Commission Accreditation was achieved in August 2000. Deering Hospital's Behavioral Health Services provided both inpatient and partial hospitalization programs. The project was started in June 2000 with a target survey date of August 2000. The scope of the project included one site, two program components, and thirty employees. Ms. Shaw was the project manager of the The Joint Commission preparation process with a staff of one.


Challenges Intensive Relapse Prevention Program

The Joint Commission Accreditation was achieved in January 2001. Challenges Intensive Relapse Prevention Program is a Full-Day and Intensive Outpatient Substance Abuse Program in Margate, Florida. This was a tri-annual survey. Ms. Potere has been an ongoing consultant for the program since the original accreditation in 1997 with a Grid Score of a Perfect 100 (See Galaxy Health Care, Inc.) This project is ongoing through 2006.


Galaxy Health Care, Inc.

The Joint Commission Accreditation was achieved in November 1997 with a Grid Score of 100. Galaxy Health Care, Inc. was a health care management company which owned a multi-site community mental health center headquartered in North Miami, Florida. Galaxy Health Care, Inc. had five community mental health centers throughout the state of Florida. The project started in January 1997 with a target survey date of November 1997. The scope included six sites throughout the state and 125 employees. Each of the five sites had two program components. Ms. Potere was the project manager of the entire The Joint Commission preparation process with a staff of one.


Homestead Behavioral Health Care, Inc.

The Joint Commission Accreditation was achieved in December 1999. Homestead Behavioral Health Care, Inc. is a community mental health center in Homestead, Florida. The project was started in May 1999 with a target survey date of December 1999. The scope of the project included one site, three program components, and twenty employees. Ms. Potere was the project manager of the The Joint Commission preparation process with a staff of one. This contact has remained ongoing since the accreditation survey. Billable services have included ongoing participation the Agency's Performance Improvement Process, development and revision of policies and procedures as requested, and development of audit tools based on specific program needs and regulatory requirements. Continued monitoring ensures compliance with The Joint Commission, DCF, AHCA, Medicaid, Medicare, and third billing recoupment.


Pathways To Recovery

The Joint Commission Accreditation was achieved in November 2003. Pathways To Recovery is an inpatient Substance Abuse Program in Delray, Florida. Ms. Potere has been an ongoing consultant for the program since July 2000. Ms. Potere and her team assisted in ongoing development of this program. This project is ongoing through 2006.


Recovery First, Inc.

The Joint Commission Accreditation was achieved in November 2003. Recovery First, Inc. is a Full- Day and Intensive Outpatient Substance Abuse Program in Oakland Park, Florida. This was an initial survey. Ms. Potere has been an ongoing consultant for the program prior to opening in July 2002. Ms. Potere and her team assisted in the entire setup and development of this program. This project is ongoing through 2006.


South Psychological & Rehabilitation Center, Inc.

The Joint Commission Accreditation was achieved in December 1999 with a Grid Score of 96 in Ambulatory Care. South Psychological & Rehabilitation Center, Inc. is a comprehensive outpatient rehabilitation facility (CORF) in Homestead, Florida. The project was started in August 1999 with a target survey date of December 1999. The scope of the project included one site and five employees. Ms. Potere was the project manager of the The Joint Commission preparation process with a staff of one.


St. Francis Community Mental Health Clinic

The Joint Commission Accreditation was achieved in July 1999. St. Francis CMHC is a community mental health center in Ft. Lauderdale, Florida. The project started in October 1998 with a target survey date of July 1999. The scope of the project included one site, two programs, and twenty employees. Ms. Potere was the project manager of the The Joint Commission preparation process with a staff of one. This contract has remained ongoing since the accreditation survey. Additional services have included ongoing participation in the Agency's Performance Improvement Process, development and revision of policies and procedures as requested and development of audit tools based on specific program needs and regulatory requirements. We provide continued monitoring to ensure compliance with The Joint Commission, DCF, AHCA, Medicaid and third billing recoupment.


The Family

The Joint Commission Accreditation was achieved in September 2003. The Family is a private psychiatric hospital in Sunrise, Florida. Ms. Potere has been an ongoing consultant for the program since February 2002, when the program opened. Ms. Potere and her team has assisted in the entire setup and ongoing development of this facility. This project is ongoing through 2006.


Transitions Recovery Program, Inc.

The Joint Commission Accreditation was achieved in August 2000. Transitions Recovery Program, Inc. is a free-standing substance abuse/eating disorder facility with residential, partial hospitalization and outpatient services in North Miami, Florida. The project was started in January 2000 with a target survey date of August 2000. The scope of the project included one site, three program components, and thirty employees. Ms. Potere was the project manager of the The Joint Commission preparation process with a staff of two. This contract has remained ongoing since the accreditation survey. Billable services have included ongoing participation in the agency's Performance Improvement Process, development and revision of policies and procedures as requested, and development of audit tools based on specific program needs and regulatory requirements. Continued monitoring ensures compliance with The Joint Commission, DCF, AHCA, Medicaid, and third billing recoupment. This project is ongoing through 2006.
